Roy,
   I use a piece of chain about 6 feet long with a hook on the end. You
take the hook and put it in the hole in the axle flange and whip it like
a whip. This will make a sudden shck pull on the axle stronger than a
slide hammer will do. 2-3 whips may be nessesary.
