As you noticed, I hedged my statement by saying that I'd never heard of
it up to now.
It was interesting to hear of your experience and I thank you for
relating it. I can now say that I've heard of at least one case where the
yoke was damaged. The geometry of that assembly is such that I'd think
that there isn't a lot of leverage on the yoke pieces where the bolts
attach. Lots of pressure, but not much leverage. So maybe it's uncommon,
but obviously as you've related, not impossible.
